This agreement is made between Daniel Lee Fitness LTD and the member whose name appears in the application for membership agreement (you).
By signing a direct debit agreement, you acknowledge that you accept these terms & conditions.
As a member you must use our Club in accordance with the following terms and conditions:
MEMBERSHIP TYPES
Please Note - all memberships are active from the day of payment until the next payment on the following month.
Group Coaching - Unlimited access to the gym on Open Sessions and access to 1-3 group sessions per week.
Open Gym - Unlimited access to the gym on Open Sessions.
Private Sessions - Unlimited access to the gym on Open Sessions as well as access to private sessions with your coach. The payment to the coach maybe done via Daniel Lee Fitness LTD or the coach individually. Membership will be paid straight to Daniel Lee Fitness LTD.
Ongoing membership types are deemed no commitment, once the minimum term has been satisifed. This just means we require notice to our email coaching@dannyleeonline.co.uk with your name to cancel.
YOUR HEALTH AND SAFETY
When you sign this Agreement and each time you use the Club, you must ensure you are in good physical condition and know of no medical or other reason why you should not exercise. If unsure, you should not use the Club until you have sought appropriate medical guidance and been given the go-ahead.
You agree to give us all relevant personal health and fitness information both before and during the course of any exercise program or other activity. You also agree to complete our Pre-Exercise Questionnaire. You promise that information you give us will be true and accurate and not misleading in any way. If you sustain any injury whilst participating in any training session with Daniel Lee Fitness or following a Daniel Lee Fitness Program, you acknowledge that Daniel Lee Fitness LTD is not responsible, except where the injury was caused as a result of negligence.
CANCELLATIONS / RESCHEDULING OF SESSIONS
To cancel or reschedule a session, a minimum of 24 hours notice is needed. If you need to cancel within 24 hours of your session, no makeup session will be given. There will be no exceptions to this policy unless there has been a medical emergency. Failure to book in owning sessions will result in a lost session. This is the clients responsibility & it must fit the Coaches schedule.
LATE ARRIVALS
If you arrive late, your 1:1 PT session will run to the agreed schedule finish time. If you arrive late to lifting club; you can participate, but the coach will finish at the agreed finish time & not stay back.
CONDUCT
You promise to take care to use the equipment safely and properly.
You acknowledge that the distribution or use of illegal or performance enhancing drugs is prohibited and promise that you will not use or distribute these substances in or near the Club. You acknowledge that engaging in any commercial or business activities in the Club, such as offering training services or selling goods in the Club is prohibited unless we grant you written permission to do so. If we do give you written permission, we can revoke this at any time. If you behave in a risky or seriously inappropriate way, for example, if you threaten or harass others, damage equipment, distribute or use illicit substances, or train other Members without our authorisation, appropriate action will be taken. For example, your Membership may be immediately suspended or cancelled, you may be or we may refer the matter to appropriate authorities. If your conduct causes us or another person costs, loss or damages you agree to pay for these.
PRIVACY
Any personal information collected will only be used to provide you with training services and will be kept confidential. We also appreciate that if athletes choose to move on from Daniel Lee Fitness, they do not disclose our training methods / programming to non Daniel Lee Fitness members. You grant us permission to use any photographs / videos taken of you during training. If you do not wish to allow us to use your photograph or videos in this way, just let us know and we will not post them publicly.
COMPLAINTS AND FEEDBACK
If you have any concerns about the Club or anything else in relation to your Membership, you should first raise it with Daniel Lee, or your own coach.
If you are uncomfortable about approaching, or do not wish to approach Club staff, or are not happy with the response given, you may send a complaint to coaching@dannyleeonline.co.uk
